Nominations should be sent to Hall of Fame, PO Box 83972, Lincoln, 68501-3972. Deadline for nominations is Oct. 1 for the next awards year.

Award is be based on the accomplishments of several members of the same family, to include one, two even three generations. Emphasis should be on the athletics prowess of the family, but can also be tied to coaching, officiating and over-all contributions to high school sports.

Use the following format to prepare a nomination for this award. This should be done in pyramid order, listing the parents under the Family Name. If listing more than one generation, organize so that persons fall under their parents.

You should begin with the parents of the nominated family. A style is provided for you to follow when preparing your nomination. Then list in order the names, relationships and accomplishments of family members as athletes, coaches, contributors.

Following would be a recommended style:

Parents' Involvement in Athletics:

Father's Name:

His Athletic accomplishments: List sports, participation, involvement, honors, etc.

Mother's Name:

Athletic accomplishments: List sports, participation, involvement, honors, etc.

Other Family Members:

Son or Daughter's Name:
Athletic accomplishments: List sports, participation, involvement, honors, etc.
Spouse:
Athletic accomplishments: List sports, participation, involvement, honors, etc.
Their Children (where applicable).
Athletic accomplishments of each: Separately list sports, participation, involvement, honors, etc.

Repeat this routine so that all sons and/other daughters and their families are included.

Finally, list the name of the person submitting the nomination, along with an address and telephone number.
